Topic 5 DQ 1
A healthcare organization`s
effective and successful operation and performance significantly depend on the
crucial multilevel barriers and factors considered as stakeholders. Stakeholders
also include corporations, individual and other agencies with special interests
in medical decision and evidence within the healthcare system where clinical
choices are developed. The support of both internal and external stakeholders
is fundamental in the successful implementation of evidence-based project in
nursing. (Franklin, 2020). This is due to the essential skills knowledge,
leadership, funds, and infrastructure necessary to implement the nursing
projects.

In every healthcare facility or organization, nurses play a
significant role in providing primary care to the patients through
evidence-based practice to ensure patient`s wellbeing. While practicing, their
activities are influenced by different internal stakeholders such as
administrator, other health care settings. The diverse providers who provide
qualitative, quantitative, and curative healthcare services to patient, thus,
directly influence care outcomes. The healthcare administration and management
which play an essential role in providing leadership and facilitating the
proposed changes` adoption, affect the implementation process (McNett,
Masciola., Sievert., & Tuckner, 2021). Therefore, for the successful
adoption and implementation of evidence-based nursing changes, collaboration
with support this internal stakeholder is crucial.
External stakeholders
such as patients, financial organizations, suppliers, insurance companies, and
the government influence the distribution and the availability of the necessary
resources and infrastructure fundamental for the proposed change
implementation. The support of external stakeholder is critical: for instance,
the government enables the development of healthcare facilities, infrastructure
and ensures training and recruitment of healthcare professionals to enhance accessibility
and affordability of healthcare services while the patient as a direct consumer
of the healthcare services vitally affect the implementation process of any
proposed nursing project. Therefore, this requires that the proposed project be
patient-centered and within the government stipulated policies and requirement.
Furthermore, the opinion of other influential individuals within and without
the medical professionals such as medial, political, and religious leaders must
also be considered (Franklin, 2020)
Implementing
evidence-based practice in nursing is a social process that requires human
interactions in the scientific decision-making process to its occurrence,
different challenges and factors come into play stages to influence the nature
of the concept, contingencies, and outcome, hence, requiring collaborators to
limit their influences. The dynamic nature of the healthcare system which
involves the participation of various stakeholders, lead to diversified aspect
of understandings, thus may lead to solid criticisms (McNett., Masciola.
Sievert & Tucker., 2021: Frankline, 2020). Support from both internal and
external stakeholders proves significant to reduce this level of complaints, losses,
and disruptions of the implementation process. Securing stakeholders’ support
for implementing the proposed change requires proper planning, continuous reminder,
and engagement to understand the essence of the change and its benefits.
Engaging stakeholders involves different activities that are not limited to
spending time and efforts to develop trust, understanding their interest,
supplicate inputs and connect to them through collaboration and positive
engagement in setting outcomes and measures within stipulated timeframes. I
will secure administrative support by explaining the project`s scope and
benefits, engaging them throughout the process, holding important meetings,
addressing their concerns about the proposed changes. Support from society and
other external stakeholders will be sought through media and community
coalitions.

According to Schoenhard (2019),
getting to know the stakeholders and gathering the majority of their support is
vital to ensure project success. Stakeholders are a wealth of knowledge about
current processes, historical information, and industry insight. Stakeholders
include the owners who have a genuine interest in the company. These people
have been at the organization or taking charge of the project longer than the
managers can be the stakeholders, too (Schoenhard, 2019). In our agency, the
internal stakeholders are the owners, the administrative executives, the
director of education, the director of nursing (DoN), and the assistant DoN.
External stakeholders are where and whom the hospice referrals come from, including
patients, families, hospital case managers, facilities like assisted living and
board and care administrators, and skilled nursing social workers.
For my project to implement
more commercial indwelling pleural catheters like PleurX or Rockett catheters
use for hospice patients with malignant pleural effusion (MPE), I need to
secure their support; otherwise, my project has more room for failure. My
vision for my change proposal is to make it readily available when needed, hire
and train more nurses for safe and effective drainage at domicile locations,
with an increased devices supply. So far, the company is limited to accepting
individuals with MPE to one-to-two in a year. I believe gathering more
evidence-based studies can help the communities that we serve without profit
loss. O’Rourke et al. (2106) suggest that sharing this vision with the
stakeholders by communicating a mental picture through the model presentation,
interviewing the enablers (the one who makes the referrals), and cancer patients’
feedbacks with data analysis can help introduce alternative ideas in the
company and eventually secure their approval. Stakeholders, in general, are
after improving healthcare systems, meeting the ever-changing demands of the
public, and aim to benefit both parties. Therefore, it is essential to include
their viewpoints; they know the clinical system longer, as I mentioned above.
Using practical strategies that lead to ways to refer to actions to influence
others, promoting the population’s quality of life, leaders will share the same
vision and do what needs to do collectively; they will protect it and ensure
that it becomes a reality (O’Rourke et al., 2016).

According to Pandi-Perumal et
al. (2015), stakeholders’ involvement is essential for management and
effectiveness in clinical research. Feedback from stakeholders provides
critical value for research managers as well as alerting them to the social,
environmental, and ethical implications of research activities. For my
research, the three key internal stakeholders are the patients, nurses, and the
facility. The three key external stakeholders are public, government and
regulatory organizations. I believe that a plan for communicating with
stakeholders must always be outlined with techniques of encouraging their
participation, and management lay out (Pandi-Perumal et al., 2015).
For me, stakeholders’ support
in the implementation of a change proposal for medication errors may yield
long-term economic gain and social values and the use of stakeholder engagement
will provide opportunities to further align clinical research practices with
societal needs, values, and expectations, thus helping to drive long-term
sustainability and stakeholder interests. The stakeholder focus group act as a
communication medium through which the opinions of individuals or groups of
individuals who are impacted by the research can be elicited such as a survey
from the nurses, and patients. In turn, external stakeholders’ support can be
obtained if the change proposal project uses existing resources to achieve the
greatest possible benefits, provide effective services to those in need, thus
improving their overall quality of life, promotes health and protects the
rights and interests of citizens. As a result, I hope that the change proposal
project through stakeholder engagement and management will promote and create a
safe environment for quality care with reduced number of medication errors. Without
the stakeholder’s support, the change proposal project would result in poor
outcome and probably an adverse effect on the patient thus contradicting a nurse’s
role.

Re: Topic 5 DQ 1
Stakeholders are vital entities that have roles
to play in the organization or those that are affected by the program or
initiatives that will be implemented in the organization. For an evidence-based
practice initiative, there are internal stakeholders and external stakeholders.
These are the individuals or organization that have direct interests in the
project, be it in the process and/or the outcomes (Boaz et al., 2018). The
participation of the stakeholders, even as early as deciding to do the project,
is essential. Support and engagement of the stakeholders is important for the
successful implementation (Boaz et al., 2018). Active engagement of the
stakeholders at multiple points in the project, from the development to
implementation to evaluation creates opportunities to have a more effective,
cost-efficient, and successful project. This is why identification of the
stakeholders is a must with every EBP initiative. As early as deciding on the
evidence-based practice (EBP) initiative, consultation with stakeholders will
help the implementors identify possible barriers and facilitators of the
proposed project (Beidas et al., 2016). These contributes to the opportunity of
addressing the barriers and increasing the facilitators prior to the project
implementation. Early engagement with stakeholders could also facilitate
obtaining funding for the project or linkage to funding sources.
Internal
stakeholders include the healthcare workers of the facilities that would
benefit from the project. Consulting with them will provide the author with
extended problem-solving resources. They will need assistance in planning, funding,
and budgeting.
Also
essential during the consultation is the timeline that shows when stakeholders
are expected to be actively involved during the implementation and the promise
that stakeholders would be updated with the progress of the project. To obtain
the support of external stakeholders, the author will organize a presentation
and distribute the feedback forms to get the hints on how the EBP proposal can
be further improved. It is vital to establish positive communication with
external stakeholders during project introduction. They must be aware of each
stage of the project implementation as well as the pits and falls that it may
cause.
Encouraging
engagement of the stakeholders is important because this would make them feel
they are part of the initiative, and their perspectives are valued. Aside from
their potential contribution of ideas, they are also more likely to present
certain perspectives that the implementors may have missed when developing and
planning the initiative. This could help in the improvement of the projects,
even within the implementation phase.

Topic 5 DQ 1
My manager, my education
department, the administration for approval of the practice changes inside the
facility, and my colleagues performing the difference with the patients are the
internal stakeholders I would need to discuss my change project. Stakeholders
play a critical role in supporting its effective implementation by contributing
their knowledge and skills and providing the necessary leadership in the
company to allow for strategic planning (Griffiths, Maggs & George, 2007).
Customers (patients) who will receive more streamlined discharge education
(results may be observed from patients who complete the HCAHPS survey) and
regulatory agencies who track readmissions are examples of external
stakeholders.
Stakeholder support is required to advocate the desired change and
has the resources (more education hours, meetings, and training) available. To
get help, you’ll need to use logic and reasoning. I’ve discussed the need for
change with my director and received her approval; the statistics from our 30-day
readmissions and HCAHPS scores will support the need for enhanced patient
discharge education in the presentation. However, my teammates’ resistance may
be mitigated by additional educational training on applying the simplified
education. They may not see the need to improve how patients are taught upon
discharge or have other feelings about the change.

Topic 5 DQ 1
Stakeholder values and concerns
reflect what stakeholders think to be important, which in turn affects how they
tend to react to the likely implications of the organization’s decisions,”
according to the Center for Evidence Based Management (Cross, 2006).
Stakeholders make the transition to change easier by persuading others. As a nurse,
you must work with internal stakeholders, such as a unit, because this is the
first stage in implementing evidence-based practice change (Cross, 2006). The
easier it is to execute a practice change when the entire unit is motivated and
engaged in it.
External stakeholders, such as the community, are also crucial in
evidence-based practice since determining the optimum implementation requires
an assessment of resources and needs (Kokemuller, 2017). The more support the
reform proposal receives, the more likely it is to succeed and set a precedent
for others. Obtaining support through education is one method of persuading
stakeholders to take an interest in the project’s implementation. Meetings,
seminars, and even emails are effective ways to spread the word about a
practice change (Kokemuller, 2017).
