DIAGNOSTIC LAB/IMAGING: Lab results showed (a)White (n)blood cells of 13. The CT scan shown (p)she has perforated (p)her (n)appendix, (a)acute (n)appendicitis.
HISTORY: The 88 year old (a)elderly (n)patient is a (a)white (n)female that showed symptoms of (a)constant pain near the (n)belly button. Per patient, (p)she stated that (p)her pain may accompany a (a)terrible (n)nausea, or (a)excruciating (n)abdominal pain when coughing that radiate to the upper (a)right (n)quadrant. Throughout the next 2 days, (p)her pain migrated to the lower (a)right (n)quadrant. The patient was admitted after 24 hours because she was not able to tolerate the pain any longer. The various diagnoses of the patient with a medical history of colitis, a stomach bacteria infection called the H pylori, and the patient was only given medication of amoxicillin for treatment. Treatment plan was discussed with the patient, (n)Dr. (n)John Doe will operate on the procedure. (p)She underwent the appendectomy (a)surgical (n)procedure, there, they evaluate and examine the perforation of the appendix. The biopsy of the tissue that was collected revealed only the inflammation and perforation but no (a)appendix (n)cancer. The appendix was removed with an intact (a)stabilize (n)line. The drain was placed to administer the right lower quadrant from bleeding. Drain is still intact.
Patient is recovering remarkably over the past 3 days of post op. No deficiency after the operation perform by Dr. John Doe. Minimal abdominal pain and she is passing gas. However, (p)her white blood cells lowered to 5. Patient is being discharged after 4 days of post op. With a 1wk gentamicin p.o, drain will be removed on follow up on 02/14/2022, and the patient is on a diet. (p)She was given a prescriiption of narcotics and (n)antibiotics.
